Job summary

Praxis Care is seeking a Team Leader at Castle Lane, Lurgan to lead a team of Support Workers in delivering person-centred care to individuals with complex needs. The role uses a key worker system to design and coordinate individual programmes and support plans.

Based on 39 hours FTE and pro rata for contracted hours, the post requires strong leadership, liaison with families and multi-disciplinary teams, effective record-keeping, and regulatory compliance.

Qualifications

• 2 years’ experience in a health and social care setting and a commitment to undertake a professional qualification.
• Qualifications in Social Work, First Level Nursing, Allied Health Profession, Level 5 Diploma in Leadership and Management for Health and Social Care or Psychology Degree and 1 year paid experience in HSC setting with commitment to qualification.
• Candidates must be prepared to complete Level 2 Safe and Effective Care Practice Diploma or already hold registration with NISCC/NMC or an Allied Health Professional.

Responsibilities

• Plan, coordinate, and implement the safe transition of new individuals into the service and community where necessary.
• Liaise with families and relevant members of multi-disciplinary teams, community groups, and voluntary bodies to promote needs.
• Plan, develop, and implement individual and group programmes of support to enhance independence.
• Coordinate ongoing assessment and review through suitable assessment models.
• Support the physical, emotional, and personal needs of the people we support with respect for dignity and independence.
• Ensure medication is held, stored, and administered according to policy and statutory procedures.
• Provide effective leadership, supervision, and management to Support Workers, including time and attendance; participate in capability, disciplinary, and grievance procedures.
• Participate in learning and development; identify development opportunities for themselves and Support Workers.
• Ensure adequate records and reports are created in line with policy and statutory requirements.
• Support the manager with regulatory compliance.
