Praxis Care in Strangford supports adults with complex needs in a living service. The Temporary Team Leader will supervise a team of Support Workers, design individual programmes, and ensure safe, person-centred care.

You will coordinate activities, liaise with families and multi-disciplinary teams, and maintain records in line with policies. Sleep-in duties may apply with additional payment.

Qualifications

• Essential: 2 years’ experience in health and social care and willingness to undertake a professional qualification; OR Social Work/First Level Nursing/Allied Health Profession/Level 5 Diploma in Leadership and Management for Health and Social Care OR 1 year paid experience in HSC with a Psychology Degree.
• Candidates must be prepared to complete Level 2 Safe and Effective Care Practice Diploma or already hold registration with NISCC/NMC or an Allied Health Professional.

Responsabilités

• Plan, coordinate and implement safe transition of new individuals into the service and community.
• Liaise with families and multi-disciplinary teams to promote needs.
• Plan, develop and implement individual and group programmes of support.
• Coordinate ongoing assessment and reviews using suitable models.
• Support physical, emotional, and personal needs with dignity and independence.
• Ensure medication is managed per policy and regulatory procedures.
• Provide leadership, supervision and management to Support Workers, including attendance monitoring.
• Participate in learning and development within the organization.
• Maintain records and reports in line with policy and statutory requirements.
• Support the manager with regulatory compliance.
