Job summary

Praxis Care is seeking a Team Leader for the Community Learning Disability Service in Portadown. The role leads a team of Support Workers to design and deliver individual and group programmes, ensuring safe transitions and strong family and multidisciplinary liaison.

Essential criteria include 2 years in health and social care and professional qualification in Social Work, Nursing, or allied health, with leadership development and regulatory compliance responsibilities emphasized.

Qualifications

• 2 years’ experience in health and social care or equivalent.
• Qualified in Social Work, Nursing, or allied health field (see essential criteria).
• Ready to pursue professional qualification and meet regulatory requirements.

Responsibilities

• Plan, coordinate, and implement safe transitions into the service and community.
• Liaise with families and multidisciplinary teams to promote needs.
• Develop and implement individual and group support programmes.
• Coordinate ongoing assessment and reviews using suitable models.
• Support physical, emotional, and personal needs with respect, privacy, dignity, and choice.
• Ensure medication is stored and administered per policy and law.
• Provide leadership, supervision, and management to Support Workers.
• Maintain records and reports per policy and statutory requirements.
• Support regulatory compliance with all regulating bodies.
