Job summary

Praxis Care in Antrim & Ballyclare is seeking a Team Leader to coordinate and lead a team delivering person-centered support. You will design individual programmes, supervise Support Workers, and ensure safe, respectful care within a 39-hour week framework.

Qualifications

• 2 years’ experience in health and social care with commitment to undertake a professional qualification.
• Qualify in Social Work, First Level Nursing, Allied Health Profession or Level 5 Leadership & Management for Health & Social Care.
• Or 1 year paid HSC experience with commitment to undertake a professional qualification and a Psychology Degree.

Responsibilities

• Plan, coordinate, and implement the safe transition of new individuals into the service and community where necessary.
• Liaise with families and multi-disciplinary teams to promote needs.
• Plan, develop, and implement individual and group programmes of support to enhance independence.
• Coordinate ongoing assessment and review using suitable assessment models.
• Support physical, emotional, and personal needs with respect, dignity and choice.
• Ensure medication is stored and administered according to policy and procedures.
• Provide leadership and supervision to Support Workers, monitor time/attendance and participate in formal procedures.
• Participate in learning and development and identify personal development opportunities.
