Job summary

Avery Healthcare’s Acer Court Care Home in Nuthall, Nottinghamshire is seeking a Team Leader who will support the home management team to deliver high-quality care in a safe, caring, and inclusive environment for residents, visitors, and staff.

As Team Leader you will lead person-centred care planning, coordinate discharges with documentation and families, monitor resident wellbeing, and uphold safe working practices. DBS clearance and eligibility to work in the UK are required.

Qualifications

  • Have an NVQ 3 or equivalent in care for care homes.
  • Two years’ experience working in a senior position with the resident group.
  • Have the ability to communicate effectively both verbally and in writing.
  • Demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Responsibilities

  • Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

ABOUT THE ROLE
