Job summary

Avery Healthcare Group is seeking a warm, enthusiastic Team Leader for Acorn Lodge Care Home in Nuneaton to support the home management team in delivering high-quality care with compassion and integrity. You will foster a safe, inclusive environment for residents, visitors and staff and drive person-centred care planning.

The role requires an NVQ 3 or equivalent in care, two years in a senior role with residents, and strong written and verbal communication.

Qualifications

Responsibilities

  • Lead the person-centred care planning process and monthly evaluations.
  • Assist in discharge transitions, including documentation and family updates.
  • Monitor residents' wellbeing and maintain safe systems of work.

ABOUT THE ROLE

Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.Other responsibilitieswillinclude:Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

  • Have anNVQ 3 or equivalent in care for care homes.
  • Two years’ experience working in a senior position with the resident group.

  • Demonstrate the ability to lead staff and act responsibly in the absence of the manager.
