Job summary

Avery Healthcare, Acorn Lodge Care Home in Nuneaton, seeks a warm Team Leader to support the home management team in delivering high-quality, compassionate care in a safe, open environment.

You will lead staff, drive care planning, and support smooth discharges while keeping residents and families informed. Requires NVQ3 in care homes and at least two years in a senior role.

Qualifications

Responsibilities

• Lead person-centred care planning by implementing, monitoring, and evaluating monthly plans.
• Assist with smooth discharge transitions, including documentation and informing residents’ next of kin.
• Monitor residents' wellbeing and update records to reflect changes in needs.
• Maintain safe working systems across tasks and moving/handling with due regard for safety.

We’re now looking for a warm, enthusiastic, and dedicated individual to join our award‑winning team as a Team Leader at Acorn Lodge Care Home in Nuneaton.

About The Role

Your focus as Team Leader will be to support the home management team in delivering high‑quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Other Responsibilities Will Include

• Leading in the person‑centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
• Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents’ next of kin fully informed, where appropriate.
• Leading with monitoring the well‑being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
• Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
