Job summary

Avery Healthcare is seeking a Team Leader for Edenbridge Manor Care Home in Edenbridge. You will support the home management team to deliver high-quality, person-centred care in a safe, open and inclusive environment for residents, visitors and staff.

The ideal candidate will hold an NVQ 3 in care for care homes, have at least two years in a senior role with residents, and demonstrate strong written and verbal communication. Applicants should be able to lead staff in the manager’s absence.

Qualifications

  • NVQ 3 or equivalent in care for care homes.
  • Two years experience in a senior position with the resident group.
  • Demonstrate effective communication both verbally and in writing.
  • Ability to lead staff and act responsibly in the absence of the manager.

Responsibilities

  • Lead the person-centred care planning process, implement monthly evaluations and audits.
  • Assist in smooth discharge transitions, including documentation and communication with next of kin.
  • Monitor wellbeing of residents and update records to reflect changing needs.
  • Adhere to safe systems of work and manage moving and handling tasks.

ABOUT THE ROLE

Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.

Other Responsibilities
  • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
