Job summary

Avery Healthcare is seeking a warm, dedicated Team Leader for Acer Court Care Home in Nuthall, Nottinghamshire. You’ll support the home management team to deliver high-quality, person-centred care in a safe, inclusive environment for residents, visitors and staff.

The ideal candidate holds NVQ 3 in care, has 2 years in a senior role, and can lead staff in the absence of the manager with strong communication skills. DBS check and UK work eligibility are required.

Qualifications

• Have an NVQ 3 or equivalent in care for care homes.
• Two years’ experience in a senior position with residents.
• Ability to communicate effectively both verbally and in writing.
• Demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Responsibilities

• Lead in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations.
• Assist in the smooth transition of any discharges from the home, including documentation and medication.
• Lead with monitoring the well‑being of each resident and ensure care plans reflect changing needs.
• Practice safe systems of work across tasks and moving and handling, prioritising safety.

ABOUT THE ROLE

Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive , and inclusive environment for residents, visitors, and staff.

Other responsibilities will include:

• Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
• Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents’ next of kin fully informed, where appropriate.
• Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
• Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
