Job summary
Nottingham University Hospitals NHS Trust is launching a surgical Virtual Ward to deliver acute-level care at patients' homes. We are seeking Senior Healthcare Support Workers to join the team, working under registered nurses across hospital and community settings to provide direct care and practical support that helps patients recover safely at home.
You will perform clinical tasks, take observations, and travel between patients’ homes and Trust sites as part of a 7-day service.
Qualifications
- NVQ Level 2 in Health and Social Care or equivalent.
- Functional Skills in Maths and English at Level 1 or above.
- Previous caring experience in health or social care setting.
Responsibilities
- Carry out nursing procedures and clinical tasks under supervision.
- Assist with personal care, comfort and hygiene in patients' homes.
- Take and record observations including NEWS2; report changes promptly.
- Conduct planned community visits following care plans and lone-working policy.
- Maintain accurate records using electronic systems and Trust handheld devices.
- Travel between hospital sites and patients' homes as part of a 7-day service model.
