Nottinghamshire Healthcare NHS Foundation Trust is seeking a Senior Community Practitioner (Nurses) to join the Nottinghamshire Virtual Ward. The role is full time and focuses on delivering hospital-level care at home, including advanced assessment and decision-making in patients' homes within a Band 6 framework.

You will work autonomously and within an MDT, support safe discharge, and develop skills in remote monitoring and digital care, contributing to system transformation and patient safety.

Qualifikationen

• Registered nurse with current NMC registration.
• Experience in community health or acute care settings.
• Ability to interpret remote monitoring data and triage appropriately.

Aufgaben

• Deliver timely, person-centred assessments for Virtual Ward patients for up to 14 days.
• Work autonomously within the MDT to prevent hospital admissions and support early discharge.
• Interpret data from remote monitoring, undertake reviews and escalate as needed.
• Lead safe discharge planning and contribute to MDT discussions on treatment and risk.
• Supervise and guide junior staff, provide clinical oversight.
• Maintain accurate SystmOne documentation and uphold governance and safety.
