Job summary

Merco Medical Staffing Limited is seeking a clinician for the Virtual Wards service in Nottingham. Work Monday to Friday, 37.5 hours weekly under a block booking arrangement, with a pay rate of £40.00 per hour and PAYE payment. A full UK driving licence and access to your own vehicle are essential as travel to patients’ homes is required.

Mileage is paid at £0.54 per mile. The role involves delivering high-quality advanced clinical care in the community and coordinating with GPs, hospital teams

Qualifications

• MSc in Advanced Clinical Practice is essential.
• Evidence of the MSc qualification must be provided.
• Appropriate professional registration and relevant clinical experience are required.

Responsibilities

• Provide high-quality clinical care to patients being managed through the Virtual Wards service.
• Carry out comprehensive patient assessments, observations and clinical reviews in patients' homes.
• Monitor patients' conditions, recognise signs of deterioration and upscale appropriately.
• Develop, implement and review individualised treatment and care plans.
• Assess and manage patients within your scope of advanced clinical practice.
• Administer and monitor treatments and medications in line with relevant policies and professional standards.
• Complete accurate and timely clinical documentation and patient records.
• Liaise closely with GPs, hospital teams, community services and other healthcare professionals to ensure continuity of care.
• Provide patients and their families or carers with appropriate clinical advice, education and reassurance.
• Support safe discharge planning and admission avoidance where clinically appropriate.
• Travel between patient homes and relevant clinical locations as required.
• Work effectively within a multidisciplinary team to deliver safe and coordinated patient care.
• Follow relevant clinical governance, safeguarding, infection prevention and health and safety procedures.
