Job summary

Royal United Hospitals Bath NHS Foundation Trust is seeking a senior nurse to lead RUH@Home, delivering person-centred care in patients' homes with complex needs. You will assess, plan and evaluate care, exercise clinical leadership, and coordinate caseloads to maintain patient flow and safe discharge.

You will work with a multidisciplinary team, ensuring safety, documentation and compassionate care while supporting ongoing improvements in the virtual ward model.

Qualifications

• Assessment, planning, delivery and evaluation of person-centred nursing care in patients' homes with acute and complex needs.
• Ability to recognise deterioration and respond or escalate appropriately.
• Deliver safe, effective and compassionate nursing care, including medicines and treatments.
• Work collaboratively with patients, families, carers and the wider MDT to promote independence and positive outcomes.
• Maintain high standards of patient safety, clinical documentation, dignity and confidentiality.

Responsibilities

• Patient Care Responsibilities: Assess, plan, deliver and evaluate person-centred care for patients with acute, complex and frailty-related needs.
• Use clinical judgement to recognise deterioration and respond or escalate appropriately.
• Deliver safe, effective and compassionate nursing care including medicines and treatments.
• Collaborate with patients, families, carers and the wider MDT to promote independence and positive outcomes.
• Maintain high standards of patient safety, clinical documentation, dignity and confidentiality.
• Clinical Leadership and Service Coordination: Provide clinical leadership, advice and support to nursing and multidisciplinary colleagues.
• Coordinate RUH@Home caseload, prioritising visits, allocating patients and supporting patient flow.
• Support management of complex and deteriorating patients with timely escalation and transfer.
• Work with acute, community and primary care partners to support admission avoidance and safe, timely discharge.
• Identify and escalate clinical or operational pressures and contribute to service improvement.
• Support effective use of remote monitoring and virtual ward technology.
