Nottingham Citycare Partnership CIC is seeking an experienced Enhanced Clinical Practitioner to join our Virtual Wards service. You will work autonomously within a city-wide model delivering acute-level care to patients in their homes, supporting admission avoidance and early discharge.

This full-time role operates on a rota from 8am to 8pm, seven days a week, across frailty, respiratory, palliative and long-term conditions, enabling MDT collaboration and ongoing service development through

Qualifications

• Substantial post-registration clinical experience.
• Experience managing acute clinical presentations and long-term conditions.
• Experience autonomously and within a multidisciplinary team.
• Experience across primary, community and secondary care settings.
• Experience of end-of-life and palliative care.
• Evidence of audit, research or quality improvement involvement.

Responsabilités

• Provide advanced clinical assessment, diagnosis and treatment for Virtual Ward patients.
• Triage referrals, coordinate care across partners, and support home-based care.
• Prescribe and review medications and manage frailty, respiratory and palliative needs.
• Act as clinical advisor, support care transitions and service redesign.
• Lead teaching, supervision and mentorship within the MDT.
