Virtual Wards – Nottingham

- Working days: Monday to Friday
- Hours: 37.5 hours per week
- Contract: Block booking
- Rate: £40.00 per hour
- Payment: PAYE only
- Vehicle: Own car required
- Mileage: Paid at £0.54 per mile
Key Duties
- Provide high-quality clinical care to patients being managed through the Virtual Wards service.
- Carry out comprehensive patient assessments, observations and clinical reviews in patients' homes.
- Monitor patients' conditions, recognise signs of deterioration and upscale appropriately.
- Develop, implement and review individualised treatment and care plans.
- Assess and manage patients within your scope of advanced clinical practice.
- Administer and monitor treatments and medications in line with relevant policies and professional standards.
- Complete accurate and timely clinical documentation and patient records.
- Liaise closely with GPs, hospital teams, community services and other healthcare professionals to ensure continuity of care.
- Provide patients and their families or carers with appropriate clinical advice, education and reassurance.
- Support safe discharge planning and admission avoidance where clinically appropriate.
- Travel between patient homes and relevant clinical locations as required.
- Work effectively within a multidisciplinary team to deliver safe and coordinated patient care.
- Follow relevant clinical governance, safeguarding, infection prevention and health and safety procedures.
Essential Criteria
- MSc in Advanced Clinical Practice is essential.
- Evidence of the MSc qualification must be available and provided as part of the compliance process in order to be considered for this role.
- Appropriate professional registration and relevant clinical experience are required.
- A full UK driving licence and access to your own vehicle are essential, as the role involves travelling to patients in the community.
- Mileage will be paid at £0.54 per mile.
