Job summary

Wye Valley NHS Trust in Hereford is seeking a Band 4 Discharge Co-ordinator to join the Integrated Discharge Team. This role focuses on patient discharge planning to ensure safe and timely discharges.

You will work with ward staff, MDT meetings, and community services, coordinating actions and communicating with patients and families to avoid delays and improve patient experience.

An induction and training opportunities are offered.

Qualifications

• NVQ/Diploma Level 4 or Foundation Degree in Health/Social Care or related experience.
• Completion of Care Certificate.
• GCSE English and Maths (or equivalent) at a good level.

Responsibilities

• Support discharge planning in partnership with IDT, ward MDT and community partners.
• Represent the IDT at ward rounds and communicate discharge actions promptly.
• Undertake delegated duties independently and report back to ward team and case manager.
• Engage with patients and families about discharge planning.
• Coordinate with community services to ensure timely discharges.
