Glassmoon Services is seeking a dedicated Supported Worker to provide compassionate, person-centred care to individuals with autism and learning disabilities. You will help with daily living, community activities, and nurture meaningful connections within their local community.

Competenze

• Experience in care or support roles is beneficial.
• Ability to support individuals with autism or learning disabilities.
• Commitment to person-centered, safe care and adherence to policies.

Mansioni

• Provide person-centered care tailored to each individual.
• Assist with daily living activities and community engagement.
• Support people to build relationships and maintain community connections.
• Contribute to the development and maintenance of Support Plans and records.
• Escalate concerns to the Team Leader or Senior Support Worker as needed.
