Job summary

Glassmoon Services is seeking a dedicated Support Worker to provide 24-hour support for individuals with autism and learning disabilities, delivering person-centred care and helping them build independence and community connections.

You’ll work with a team that values person-centred planning, collaboration with professionals, and progression opportunities while supporting a range of needs and presentations.

Responsibilities

• Deliver person-centred care aligned with our policies and procedures.
• Help individuals engage with their interests, maintain skills, and participate in the community.
• Maintain and update Support Plans and related information to support wellbeing and independence.
• Notify the Team Leader of any changes in each individual’s needs to ensure ongoing care.
• Safeguard the welfare of those in our care and promote their safety at all times.
• Assist individuals to build relationships with family, friends and other important people in their lives.
