Job summary
Dignus Group is seeking a dedicated Support Worker to help adults with complex needs live independently and achieve personal goals. You will work to individual care plans, assist with daily living tasks and support participation in community activities.

Qualifications
- Experience supporting vulnerable adults or similar roles.
- Ability to follow individual care plans and instructions.
- Strong record-keeping and organisational skills.
Responsibilities
- Support with daily living tasks such as cooking, cleaning and shopping.
- Develop life skills and confidence.
- Support access to community activities and opportunities.
- Encourage choice, independence and informed decision-making.
- Support individuals to maintain their tenancy and home environment.
- Complete accurate daily records and support documentation.
- Work with social workers, families and wider networks.
- Assist with transitions, including admissions and moves between services.
- Respond calmly to incidents, risks and emergencies.
- Create a safe, positive home environment where people feel valued.
