Job summary

Langley Trust in Ashdene, Wakefield invites applications for a permanent, full-time Support Worker. You will deliver person-centred care, maintain client records and support independence across 37.5 hours per week (3 x 12.5 hour shifts), with some weekend work.

You will provide personal care, assist with mobility, monitor risks, follow medication procedures and participate in team meetings to ensure a safe, supportive service for our clients.

This is a permanent, full‑time position working 37.5hours per week, where you’ll play a meaningful role in supporting our clients and contributing to our mission. KEY RESPONSIBILITIES

• Provide keywork support (including acting as Key Worker where required), maintaining accurate client records and using the Langley Client File to review progress and promote independence.
• Contribute to a safe, well-run service by following medication procedures, supporting participation and client meetings/involvement (including complaints), supporting prospective/former residents, maintaining safe/tidy environments, and participating in rotas and team meetings.
• Provide personal care as identified in care plans (including supporting with hygiene, dressing, continence care, mobility/moving and handling, and supporting access to health and social care professionals) and keep health-related records and care plans up to date.
• Assess and manage risk and challenging behaviour by using; planning, de-escalation, and knowledge of our clients. Maintain service security and situational/physical safety measures, liaising with relevant stakeholders as needed.
