Job summary

Agincare is seeking an Outreach Support Worker to assist individuals in Bexley with Learning Disabilities, Autism, Mental Health needs, and complex support requirements. You will provide person-centred support in their homes and local community, helping them achieve goals and maintain meaningful connections.

Responsibilities

  • Encouraging people to play active roles in their communities and promote independence.
  • Support daily living activities including social activities, personal care, routines, meals and budgeting.

  • Engage to enhance quality of life for the people we support.
  • Specialise in supporting people who may need positive behavioural support approaches.
  • Record accurate information and report changes in health or circumstances to management.
  • Work as part of a team and also use your own initiative.

As an Outreach Support Worker, you will provide high-quality, person-centred support to individuals with Learning Disabilities, Autism, Mental Health needs, and complex support requirements, helping them achieve their personal goals, develop life skills, and maintain meaningful connections within their community.

Where you’ll be working:

Bexley, Bromley & Newham

You will provide outreach support to individuals within their own homes and local community.

As part of your day to day work you will be offering the following;

  • Supporting – Activities of daily Living including social activities, personal care, routines, planning and preparation of meals and budgeting.

  • Engaging - promoting and motivating people we support to enhance their quality of life
  • Specialising - supporting people who may need positive behavioural support approaches
  • Recording - keeping accurate records &reporting changes in health or circumstances of service users to management
  • Team player– Working as part of a team, as well as being able to work using your own initiative.
