Job summary

Agincare Group is seeking a dedicated Mental Health Support Worker to join the Bromley Borough Floating Support team. You will provide outreach support to individuals in their own homes and local community, helping them maintain independence and improve quality of life.

You will support people with mental health needs and other complex needs, work as part of a team, and contribute to positive outcomes.

Qualifications

• Experience in community-based mental health support or similar role.
• Ability to work in people’s homes and local community.
• Committed to promoting independence and wellbeing.

Responsibilities

• Provide outreach support to individuals in their own homes and community.
• Promote recovery, independence and wellbeing throughout the mental health journey.
• Encourage active participation in communities to support independence.
• Assist with activities of daily living including meals, routines and budgeting.
• Communicate effectively using the individual's preferred method.
• Record changes in health or circumstances and report to management.
• Work as part of a team and on own initiative.

We are looking for a dedicated and enthusiastic Mental Health Support Worker to join our Bromley Borough Floating Support team.This role focuses on supporting individuals living in the community who experience mental health needs, helping them to maintain independence and improve their quality of life.

You will need to have excellent people skills, a positive and proactive approach, and a genuine passion for supporting individuals with mental health needs, alongside other complex needs which may include learning disabilities or autism.

You will provide outreach support to individuals within their own homes and local community.This includes working with people at different stages of their mental health journey, promoting recovery, independence, and wellbeing.
