Job summary

Outward in London is seeking a proactive Support Coordinator to join our team supporting adults with learning disabilities, autism, mental health needs and complex support requirements in our supported living services.

You will empower people to live independently by providing high-quality, person-centred support and coordinating essential activities such as support planning, rota oversight, and staff guidance.

Qualifications

• Experience supporting adults with learning disabilities, autism, or mental health needs in supported living.
• Strong safeguarding knowledge and risk management.
• Knowledge of health and safety in 24-hour supported living.
• Experience with key working and support plan reviews.
• Ability to guide staff and cover shifts as required.
• Confidentiality, GDPR and data protection understanding.

Responsibilities

• Provide direct, person-centred support promoting independence and dignity.
• Coordinate day-to-day service delivery ensuring plans are followed.
• Lead key working, review support plans and risk assessments.
• Oversee shift coordination and rota management for safe staffing.
• Mentor staff and provide line management where delegated.
• Conduct medication checks and safe administration monitoring.
• Monitor petty cash, receipts, and basic financial checks.
• Report maintenance issues and ensure health and safety standards.
• Maintain accurate daily records and digital care documentation.
• Work evenings and weekends to ensure consistent service delivery.
