Job summary

Creative Support in Doncaster is seeking a compassionate and dynamic Support Coordinator to join our Supported Living Service. You will coordinate support for 28 individuals across three services, working with families and professionals to help people live independently with high-quality, person-centred care.

You will develop and monitor support plans, build relationships, promote independence, and support staff to deliver compassionate, outcome-focused support while enabling community

Qualifications

• Experience in social care or supported living is preferred.
• Ability to work with families and external professionals.

• Respond proactively to changing needs while promoting positive risk-taking and effective risk management.

We are looking to welcome a caring, dynamic and highly motivated Support Coordinator to join our well-established Supported Living Service in Doncaster. As a Support Coordinator, you will play a key role in coordinating support for 28 individuals across 3 supported living and outreach services in Doncaster. Working alongside colleagues, families and external professionals, you will ensure people receive consistent, high-quality support that enables them to achieve their goals and live meaningful lives.

Key Responsibilities

• Support and coordinate services for 30 individuals with learning disabilities and mental health needs.
• Develop, review and monitor person-centred support plans and risk assessments.
• Build positive relationships with the people we support, their families, advocates, and external professionals.
• Ensure that support plans promote independence, strengths, aspirations, and positive outcomes.
• Monitor service quality to ensure consistently high standards.
• Support staff teams to deliver compassionate, outcome-focused support.
