Job summary

Outward is seeking a proactive Support Coordinator to join our team at Desmond Court, supporting adults with learning disabilities, autism, and mental health needs. You will balance hands-on care with service coordination and leadership responsibilities to ensure safe, high-quality day-to-day operations.

You will lead shift coordination, mentor staff, and oversee care plans, safeguarding, and documentation, while working across multiple services in a supportive living environment.

Qualifications

• Experience supporting adults with learning disabilities, autism or complex needs.
• Strong safeguarding and risk-management knowledge.
• Knowledge of health and safety in a 24-hour supported living environment.
• Experience with key working, support plans and outcome reviews.
• Ability to guide and supervise staff and maintain professional standards.
• Excellent communication and organisational skills.

Responsibilities

• Provide direct, person-centred support to people we support, promoting independence, dignity, choice and wellbeing.
• Coordinate day-to-day service delivery, ensuring planned, responsive support.
• Carry out key working, including support plans, risk assessments and reviews.
• Lead on shift coordination and rota oversight to maintain safe staffing levels.
• Support staff practice with mentoring and role modelling good care.
• Provide line management and supervision to support staff, including inductions and appraisals.
• Carry out routine medication checks, MAR reviews, stock counts and escalation as needed.
• Support financial monitoring with basic checks of petty cash and receipts.
• Report and monitor maintenance issues with relevant teams to maintain safety and standards.
• Ensure accurate, timely documentation on daily records, incidents and safeguarding logs.
• Work flexibly across services, including evenings and weekends for continuity.
