Job summary

Creative Support seeks an experienced practitioner to join our Headington, Oxford service as a Support Coordinator. You will deliver high-quality, person-centred care to individuals with learning disabilities, physical disabilities, and mental health needs to live independent lives.

You will supervise an established team, coordinate delivery of support, and empower tenants to be central in decisions. The post involves flexible shifts, weekends and occasional sleep-ins; benefits include a

Qualifications

• Experience in social care with learning/physical disabilities and mental health support needs.
• NVQ3/HSC Diploma Level 3 or equivalent required; willingness to train to Level 5.

Responsibilities

• Supervise an established staff team delivering person-centred support.
• Coordinate, manage and coach the staff team.
• Deputise for the Service Manager.
• Engage with families and professionals to promote independent living.
