Job summary

Home Group Limited is recruiting for a Support Coordinator to lead the planning of support and coordinate a team of Support Workers to deliver person-centred services in Bodmin. The role focuses on enabling customers with complex needs to live more independently while balancing safeguarding, admin, and team collaboration.

You will help customers with daily tasks, work with multi-agency teams, and guide individuals toward greater independence and eventual move-on into the community.

Qualifications

• Experience supporting others in housing or community settings.
• Level 3 Diploma in Care or equivalent experience (or willing to work towards it).
• Experience assessing referrals and coordinating support; able to update care plans using technology.

Responsibilities

• Lead support planning and coordinate our Support Workers to deliver person-centred support.
• Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support Customers with daily tasks like budgeting, cooking and attending appointments.
• Work with multi-agency teams to safeguard and empower our customers.
• Help customers build independence and prepare for their move-on into the community.
