Sunderland Care and Support Ltd in Sunderland district is recruiting a Care Co-ordinator (Level 1) for the Mental Health Team, based on-site in Houghton-le-Spring. The role offers 37 hours per week with a starting salary of £27,274 to £29,071 per year.
You will work closely with people using our services to promote independence, dignity and wellbeing, delivering person-centred care and supporting with daily living, meals, medication and community involvement.

We are seeking to recruit a Care Co-ordinator (Level 1) into our Mental Health Team who can deliver exceptional standards of person-centred care, highlighting co-production approaches that enable people who use our services to have as much choice and control as possible over their own care and support needs.
This newly designed model of care will provide an assessment pathway for individuals with severe mental ill health. The support provided will also include intensive rehabilitation, which aims to maximise an individual's quality of life and social inclusion by developing their skills, promoting independence and autonomy, and giving individuals hope for the future. This approach supports successful community living through appropriate assessment and support.
The successful candidate will support customers to maintain and build fulfilling lifestyles, promoting independence, dignity, respect, engagement, and participation in all aspects of daily living. You will work closely with customers to achieve positive outcomes, encourage personal development, and ensure that their health, wellbeing, and quality of life are always prioritised.
