Job summary

Home Group in the United Kingdom is seeking a dedicated Support Coordinator to lead planning, coordinate support workers and deliver person-centred care. You will balance safeguarding, admin and meetings while guiding colleagues and managing caseloads.

You will help customers with daily tasks, work with multi-agency teams, safeguard customers and support their independence as they move on into the community.

Qualifications

• Passion and experience in supporting others, ideally in a supported housing setting.
• Level 3 Diploma in Care or equivalent experience (or willingness to work towards it).
• Experience assessing referrals and coordinating support.
• Ability to work on your own initiative, remain calm under pressure and have a resilient approach.

Responsibilities

• Lead support planning and coordinate our Support Workers to deliver person-centred support.
• Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support Customers with daily tasks like budgeting, cooking and attending appointments.
• Work with multi-agency teams to safeguard and empower our customers.
• Help customers build independence and prepare for their move-on into the community.
