Job summary

Home Group in the Tees Valley is seeking a compassionate care lead to help people with complex needs live more independently. You will coordinate Support Workers, manage caseloads, and ensure person-centred plans across services.

The role combines field and home-based work in a hybrid setting, requiring flexibility for early mornings, days, and evenings. A Level 3 Diploma in Care or willingness to pursue it is essential, with collaboration across multi-agency teams and daily tasks like budgeting

Qualifications

• Passion and experience in supporting others, ideally in a supported housing setting.
• Level 3 Diploma in Care or equivalent experience (or willing to work towards it).
• Experience assessing referrals and coordinating support.
• Ability to work under your own initiative, stay calm under pressure and have a resilient approach.
• Willingness to work across services in the area.

• 5 week rolling rota, varying shifts covering early mornings, days and evenings
• Availability to work waking nights and sleeps as required
• Hybrid role working in services and at home
• Although you'll lone work, we do get together regularly for training and team meetings.
• You need an Enhanced with barring list (we pay)
• A vehicle insured for business use (and a license!), we'll pay for any business mileage
