We are recruiting a Support Coordinator for our Floating Support Service in Gateshead that provides outcome-focused care and support to individuals who are at risk of homelessness and in need of housing related provision. You will be an active part in supporting individuals with complex needs and requirements, helping them access facilities to find and maintain tenancies in the local community. At our Floating Support Service, we promote inclusion, independence and wellbeing with the aim of enabling the people we support to become active and productive in their personal lives.
This role requires you to manage a team of recovery workers and provide direct support to service users in accordance with their holistic support plans. This will include providing one-off interventions as well as longer term support, devising clear exit strategies that will help service users build resilience, local connections and provide them with the knowledge of local service provision.
Experience is essential for this role, as well as access to a car.
You will be expected to work flexibly to meet the needs of the service, including evenings and weekends.
