Job summary

Waythrough is seeking a dedicated Support Coordinator to provide floating support and housing management for adults facing multiple disadvantages in the Bradford District. You’ll work in clients’ homes and the community to help sustain accommodation, enhance wellbeing, and support independent living.

The role requires an NVQ Level 3 in Health & Social Care (or willingness to pursue), knowledge of welfare benefits, safeguarding, and multi-agency collaboration.

Qualifications

  • NVQ Level 3 in Health & Social Care (or equivalent) or willing to work towards it.
  • Knowledge of welfare benefits system and relevant legislation.
  • Knowledge of housing options and relevant legislation.
  • Knowledge of Safeguarding practices and relevant legislation.
  • Experience of multi-agency working with external partners.
  • Experience of supporting vulnerable people and risk management.
  • Ability to manage a caseload and support independence.
  • Good record-keeping and use of case management systems.

Responsibilities

  • Provide high-quality, person-centred floating support and housing management to adults with multiple disadvantage across the Bradford District.

  • Deliver housing-related support to establish and sustain tenancies and reduce homelessness.
  • Collaborate with partner agencies to provide coordinated support and manage risk.

  • Develop and review support/risk plans and carry out needs assessments and risk assessments.
  • Undertake outreach to referral agencies and stakeholders to publicise the service and receive referrals.
  • Record case notes and update information in databases for monitoring and evaluation.

Location: 11-12 Eldon Place, Bradford, BD1 3AZ

Working Hours: 37 hours per week, 9am-5pm Monday-Friday with evening and weekend work included when required

Contract Type: Permanent

Salary: £25,877.80 - £34,240.03 per annum

About The Role:

To provide high-quality, person-centred floating support and housing management to adults with multiple disadvantage across the Bradford District.

Working alongside individuals in their own homes and within the community, the role will empower people to sustain their accommodation, improve their wellbeing, build independence and reduce the risk of homelessness through person-centred, strengths-based, trauma-informed and outcome-focused support.

What You'll Do:
  • Support clients to live independently in their own accommodation and progress towards sustained independent housing.
  • Deliver person-centred, strengths-based and trauma-informed support, enabling individuals to achieve positive outcomes, improve wellbeing and maximise their independence.
  • Provide housing-related support to help clients establish and sustain tenancies, prevent homelessness and access appropriate services within the community.
  • Work collaboratively with partner agencies to deliver coordinated support, manage risk effectively and ensure individuals receive the right support at the right time.
  • Work as part of a team providing cross provision/ service support where reasonable and attending regular team meetings.
  • Manage a caseload of service users as allocated by the Service Manager and complete high quality and comprehensive needs assessments and risk assessments for those service users.
  • Develop comprehensive support / risk plans to identify how individual service user needs will be met to help each service user live independently within the community and achieve sustained outcomes.
  • Undertake outreach sessions to referral agencies and other stakeholders to publicise the service, to receive referrals and to give signposting advice. To attend any relevant multi-agency meetings as required.
  • Review individual support / risk plans within appropriate timescales and no later than once every three months.
  • Record case notes, input information onto the database promptly and maintain other relevant Management Information Systems as required in order to ensure effective case management and assist monitoring and evaluation of service delivery.
  • Work flexibly across operational sites and within an agreed number of hours to maintain appropriate service provision, This will include evening and weekend working.
To Succeed in This Role, You'll Need:
  • Essential:
  • NVQ Level 3 in Health & Social Care (or above or equivalent) or willing to work towards this in post.
  • Knowledge of welfare benefits system and relevant legislation.
  • Knowledge of housing options available and relevant legislation.
  • Knowledge of Safeguarding practice and relevant legislation.
  • Experience of multi-agency working with a wide range of external partners.
  • Experience of supporting vulnerable people, including support planning and risk management.
  • Experience of, and ability to, manage a case load of service users, supporting them to achieve greater independence.
  • Ability to communicate well and maintain accurate records within appropriate case management systems.
  • Ability to drive and owns a vehicle which can be used for work purposes.
  • Proficient in Microsoft Outlook Office programmes.
