Job summary

Bridgend County Borough Council is seeking a motivated Discharge Co-ordinator to join the Support at Home Team. This role combines hospital discharge planning with coordinating community referrals into Short-Term Assessment and Reablement Services, supporting people to live safely and independently at home.

You will work with individuals, families, health professionals and local partners to ensure timely, person-centred support.

We are seeking a motivated, enthusiastic, and committed individual to join our established Support at Home Team. This role combines hospital discharge planning with the coordination of community referrals into Short-Term Assessment and Reablement Services, supporting people to live safely and independently at home.

About the Role

Working closely with individuals, families, health professionals, social care providers, and community partners, you will:

  • Support safe and timely hospital discharges using the Discharge to Recover and Assess model.
  • Coordinate community referrals into Short-Term Assessment and Reablement Services to promote independence and prevent unnecessary hospital admissions.
  • Work collaboratively with multidisciplinary teams to ensure coordinated, person-centred support
  • Monitor and manage referrals, assessments, and service capacity to ensure timely interventions.
  • Identify risks associated with discharge and community living arrangements.
  • Maintain accurate records and contribute to the continuous improvement of the service.
