Job summary

Bridgend County Borough Council is seeking a motivated Discharge Co-ordinator to join the Support at Home Team. This role combines hospital discharge planning with coordinating community referrals into Short-Term Assessment and Reablement Services, supporting people to live safely and independently at home.

You will work with individuals, families, health professionals and local partners to ensure timely, person-centred support.

• Experience in health, social care, reablement and community services.
• Excellent communication and strong organisational skills.
• Ability to build positive relationships with individuals, families and professionals.
• Ability to prioritise workload in a fast-paced environment.
• Knowledge of person-centred practice, safeguarding and social care legislation.

Responsibilities

• Support safe and timely hospital discharges using the Discharge to Recover and Assess model.
• Coordinate community referrals into Short-Term Assessment and Reablement Services.
• Work with multidisciplinary teams to ensure coordinated, person-centred support.
• Monitor and manage referrals, assessments and service capacity.
• Identify risks associated with discharge and community living arrangements.
• Maintain accurate records and contribute to service improvement.
