Job summary

Homelessness is an increasing public health challenge impacting the health and well-being of children and their families placed in temporary housing. Children and their families are often displaced from their communities and schools. Boroughs may have multiple locations where homeless families are housed. The post holder will work in partnership to meet the needs of homeless children and families, aiming to reduce health inequalities and improve outcomes, especially during the first 1001 days and until they reach school age. Safeguarding children and young people at risk of harm is a key part of this role. The post holder will act as a resource for the mainstream 0-19 service, providing expert advice and guidance, and advocate for homeless families with stakeholders, particularly local authorities and MPs. The role also involves influencing policies affecting the health and wellbeing of residents in homeless and temporary accommodation.

Main duties of the job

  1. Manage an enhanced caseload of families residing in temporary accommodation.
  2. Provide the 0-5 Healthy Child Programme and 5-19 Healthy Child Programme, liaising with designated school nurses.
  3. Collaborate with local statutory and mainstream services to empower families to access health and support services confidently.
  4. Initiate, plan, and coordinate care for children and families in consultation with other professionals.
  5. Adhere to CLCH's clinical standards.
  6. Participate in evidence-based practice and audits to develop good practices, optimize resource use, and maintain standards.

Person Specification

Qualifications

  • Registered Nurse/Registered Midwife
  • Registered Health Visitor (SCPHN)
  • Current NMC registration
  • Evidence of ongoing post-registration education and training.

Desirable

  • Practice assessor/supervisor training or working towards it.

Experience

  • Health visiting experience with managing complex and safeguarding situations.
  • Ability to coordinate health promotion initiatives aligned with health improvement targets.
  • Experience working collaboratively within multidisciplinary and inter-agency teams.
  • Experience in workload management and care planning.
  • Experience in evidence-based care.

Skills

  • Knowledge of recent NHS policy developments.
  • Understanding of current health and social care agendas and legislation, including child protection issues.

Key Attributes

  • Strong communication skills, both written and verbal.

Desirable

  • IT skills proficiency.
