Specialist Health Visitor - Homeless Health Visitor

Role Overview

Homelessness is an increasing public health challenge affecting children and families in temporary housing. The post holder will collaborate with partners to address their needs, reduce health inequalities, and improve outcomes, especially during the first 1001 days of life and until school age. The role includes safeguarding children at risk, providing expert advice to the 0-19 service, advocating for homeless families, and influencing policies related to health and wellbeing in temporary accommodations.

Key Responsibilities

1. Manage an enhanced caseload of families in temporary accommodation.
2. Deliver the 0-5 and 5-19 Healthy Child Programmes, liaising with school nurses.
3. Collaborate with statutory and mainstream services to empower families to access health support.
4. Plan and coordinate care with other professionals.
5. Adhere to CLCH's clinical standards.
6. Engage in evidence-based practice and audit to maintain high standards.
