Job overview

JOB PURPOSE: Monday-Friday

To plan, organize and evaluate an innovative health visiting service, and where appropriate, delegate to other members of the health visiting team to ensure delivery of a high quality service.

To provide a skilled, effective and evidence-based health visiting service to clients in a variety of community settings.

To work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities.

To collaborate with team colleagues in order to deliver the Quality Strategy objectives.

Main duties of the job
  • Deliver the healthy child programme and work in partnership with families to develop and agree tailored health plans as required.
  • Utilise specialist skills and knowledge to advise and support individuals and families on evidence-based health care practices.
  • Understand the caseload profile including the health needs of the defined population and local communities, targeting outreach services to meet their needs.
  • Initiate referrals to multi-disciplinary/agency colleagues as required to ensure adequate care and support for families and children.
  • Assess, plan, implement and evaluate individual/group programmes of care that meet the public health needs of the population.
  • Develop and delegate appropriately the implementation of regular health promotion initiatives on specific health issues and to specific groups and individuals.

  • Provide a skilled, effective, and evidence-based health visiting service to clients in community settings.
  • Apply specialist skills and knowledge in delivering the health visiting service.
  • Consult, advise, and educate individuals and families on good health care practices.
  • Develop health profiles of the population and local communities, identifying health needs of disadvantaged groups and targeting outreach services.
  • Identify, prioritise, and implement programmes of care that meet public health needs.
  • Provide parenting skills support through groups and home visits, especially for those with complex needs.
  • Conduct health assessments, advice, and education tailored to local public health needs.
  • Implement health promotion initiatives on specific health issues.
Person specification
Education
  • Registered Nurse or Registered Health Visitor with current NMC registration.
  • Evidence of continuing post-registration education and training.
Experience
  • Recent health visiting experience.
  • Ability to coordinate health promotion initiatives.
  • Experience working collaboratively in multi-disciplinary teams and inter-agency settings.
  • Experience managing own workload and assessing needs.
  • Mentor training or willingness to undertake it.
