County Durham & Darlington NHS Foundation Trust is seeking a Specialist Clinical Pharmacist to join the pharmacy team on a rotational basis, across Darlington Memorial Hospital and University Hospital of North Durham. The role supports medicines optimisation, governance, education and service development with mentoring and prescribing development opportunities.

Qualifications

• Masters Degree in Pharmacy or equivalent.
• Postgraduate clinical qualification or working towards diploma level.
• Evidence of Continuing Professional Development (CPD).
• Pharmacist prescriber or training to commence within 12 months.

Responsibilities

• Deputise for the Pharmacist Advanced and supervise a team to deliver clinical pharmacy services.
• Provide medicines management, medicines optimisation and reconciliation on designated wards/clinics.
• Develop and implement local policies and prescribing guidelines.
• Influence practice to ensure safe medication use and risk minimisation.
• Be a point of contact for nursing and clinical staff on medicine management.
• Provide medicines information and assess new drug reviews.
• Support clinical pharmacy services within MDT structures (outpatients/pre-assessment).
• Deliver high quality patient education on medicines use.
• Contribute to clinical effectiveness: audits and service evaluation.
• Assist planning and implementation of service changes and new treatments.
• Act as a professional checker in dispensary/chemo services.
• Participate in weekend, on-call, late-night and bank holiday rotas.
