Job summary

Primary Healthcare Darlington Limited is recruiting a Clinical Pharmacist to join the Enhanced Health in Care Home (EHICH) Team within the PCN. You will support Medicines Optimisation, conduct Structured Medication Reviews, and work with Care Home residents and GP practices to optimise treatments.

The role involves independent prescribing or commitment to training, with close collaboration across the PCN and NHS services. 37.5 hours (flexible to 30 hours).

Qualifications

• Proven ability to undertake structured medication reviews and optimise medicines.

Responsibilities

• Lead Structured Medication Reviews in monthly Care Home MDT.
• Deliver medicines optimisation across care homes and GP practices.
• Provide clinical supervision to Pharmacy Technicians.
• Support prescribers with complex polypharmacy and co-morbidity management.
• Ensure practice compliance with antimicrobial stewardship guidance.
