Job summary

East of England Community Health & Care NHS Trust is seeking a pharmacist to work within general practice, supporting and mentoring with a senior clinical pharmacist. You will handle chronic disease management, clinical medication reviews and medicines optimisation as part of a multi-disciplinary team.

The role involves collaboration with community and hospital pharmacy teams, and there is potential to develop into non-medical prescribing.

Qualifications

• Undergraduate degree in pharmacy and GPhC registration.
• Minimum 2 years' experience as a pharmacist in practice.
• Aware of common acute and long-term conditions in general practice.
• May hold or be working towards independent prescribing.

Responsibilities

• Conduct patient-facing clinical medication reviews and medicine optimisation.
• Support repeat prescribing and medicines reconciliation on transfer of care.
• Provide medicines information to staff and patients and contribute to quality improvement.
