Cambridgeshire County Council is seeking a qualified social worker to lead Care Act assessments within the Prevention and Early Intervention Assessment team. You will manage a caseload and contribute to early access to appropriate services, while supporting unpaid carers.

You will join a team prioritising safeguarding and collaborative case discussions, with a duties rotation to screen referrals and ensure timely responses to needs.

Qualifications

• Qualified social worker
• Enhanced DBS check
• Commitment to safeguarding children and vulnerable adults

Responsabilités

• Hold and manage a caseload of Care Act assessments
• Assess people referred from reablement teams who have longer-term care and support needs
• Support unpaid carers through strengths-based conversations and planning
• Help individuals access the right support at the earliest opportunity
• Take a rotating turn on duty, screening referrals into the team
• Support team members through case discussions
