Job summary

Cambridgeshire County Council is seeking a qualified social worker to join the Adult Early Help team in a full-time, permanent role. You will promote independence, wellbeing and positive outcomes for adults across Cambridgeshire, working with a multidisciplinary team to provide timely assessments and appropriate pathways.

Qualifications

• Qualified Social Worker with current Social Work England Registration.
• Good working knowledge of the Care Act 2014 and Mental Capacity Act 2005.
• Experience of multi-disciplinary working to enable best outcomes for individuals.
• Strong communication and organisational skills, and ability to work independently.

Responsibilities

• Triaging referrals in adult early help and acting within a front door team.
• Developing community action plans and supporting unpaid carers.
• Contributing to safeguarding referrals and assessing risks to adults.
• Using strengths-based approaches and coordinating with health and prevention services.
• Signingpost to preventative and community services and supporting Care Act principles.
