Job summary

Cambridgeshire County Council invites applications for a Qualified Social Worker to join the Prevention and Early Intervention (PEI) Assessment team in Cambridgeshire. You will manage a caseload, support duty rotas, and undertake Care Act assessments and carer conversations within a diverse community.

The role focuses on preventing, reducing and delaying care needs, with opportunities for professional development and progression. Enhanced Disclosure through DBS required.

Qualifications

• Qualified Social Worker with current Social Work England registration.
• Strong knowledge of the Care Act 2014 and Mental Capacity Act 2005.
• Experience working in multi-disciplinary teams to achieve outcomes.
• Excellent communication, planning and organisational skills, and ability to work under pressure.

Responsibilities

• Complete adult social care assessments.
• Support unpaid carers through conversations and assessments.
• Develop care and support plans and carer support plans.
• Contribute to safeguarding referrals and assess risks to vulnerable adults.
• Apply Mental Capacity Act principles, including capacity assessments.
• Signpost to preventative and community services and enable timely support.
