Job summary

Greater Peterborough Network is seeking a coordinator for its hospital-at-home service. The role centers on handling inbound referrals, answering patient and family queries, and coordinating follow-ups and discharges with hospital teams and GP practices.

The service operates 8am–8pm, seven days a week on a rota basis. GCSE English and Maths or equivalent are required, with strong IT and communication skills to support robust patient records and effective teamwork.

Qualifications

• GCSE English and Maths or equivalent.
• Proficient in MS Office including Excel, Word and Teams.
• Strong written and verbal communication.
• Ability to plan and organise own workload.
• Excellent interpersonal and collaborative skills.

Responsibilities

• Handle inbound referrals and act on them promptly.
• Respond to queries from patients and family members.
• Coordinate follow-ups and discharges with clinical teams.
• Update patient records with care plans during MDT meetings.
• Liaise with hospital teams and GP practices about patient care.
• Report incidents, complaints and escalations to the Service Manager.
