Qualifications

• NVQ Level 3 or equivalent or working towards this.
• GCSE Maths and English (C or above) or equivalent

Responsabilités

• Identify and manage a caseload of patients with personalised care plans.
• Coordinate patient healthcare with GPs and other clinicians to ensure appropriate services.
• Assist patients in making appointments and accessing education, training, employment, and benefits.
• Promote shared decision-making and provide information to support informed choices.
• Support MDT coordination within the PCN and manage referrals via NHS App.
