We are very excited to be able to offer a Full time permanent Social Work post within the Assertive Outreach Team in Stroud based in Weavers croft. The post is between the hours of 9-5 Monday to Friday with occasional unsocial hours when needed. We are seeking an experienced Social Worker who is used to care management, working with complex mental health patients. Weavers Croft is a community base on site with other community teams to enable helpful information/ skills/ knowledge sharing and positive interface working.
The Assertive Outreach Team is a friendly, welcoming and dynamic team providing secondary care services to adults of working age who have complex needs and are reluctant to engage with mental health support services. You will offer support to their families and significant others, to provide the best possible outcomes for people experiencing psychosis, substance/alcohol use and are high risk of harm to self or other. The team is currently under review in alignment with intensive and assertive outreach review governed by NHS England. The service overall has received new recurrent funding making this an exciting time to join an expanding service where we can influence how to achieve the best outcomes for service users. We strive to ensure that locality services represent excellent quality, (safe and a positive experience) and good value, (efficient and effective).
The post will include care coordinating a designated caseload, working within CPA Policy and associated countywide risk procedures, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the MHA and MCA.
