Job summary

Proclaim Care Limited is seeking Senior Rehabilitation Case Managers to lead regional teams from a home-based base in the Exeter area. You will guide clinical decision-making, coordinate complex case management, and collaborate with solicitors, insurers and employers to optimise client recovery and return to work.

The role requires HCPC or NMC registration and a strong background in rehabilitation case management, with travel across the region and a commitment to high‑quality, client‑centred

Qualifications

• Qualified healthcare professional with HCPC or NMC registration.
• Proven leadership and line management experience.
• Strong decision‑making skills within a governance framework.
• Willingness to travel and hold a valid driving licence.

Responsibilities

• Lead operational teams and provide line management to Rehabilitation Managers.
• Coordinate rehabilitation programmes across private and statutory services.
• Oversee client assessments, treatment coordination and reporting.
• Support growth and ensure financial targets and SLAs are met.
• Travel regionally to work with clients and partners.
