Job summary

Surrey Downs Health and Care is seeking a Senior Rehabilitation Support Worker to join the Recovery At Home team. You will help deliver community and hospital-based rehabilitation, supporting discharge, and ensuring patient-centered care in a Flexible 7-day service.

You will assess needs, plan care, coordinate services, and work within a multidisciplinary team to promote independence and safe, timely care. A driving license and NVQ 3 or equivalent are required.

Qualifications

• Educated to NVQ 3 level or equivalent in a relevant subject or significant equivalent experience.
• Intermediate knowledge of IT systems, e.g. Outlook, Word, Excel; AIS/EMIS knowledge mentioned in person spec.

Responsibilities

• Complete skilled screening assessment for clients referred to the service, prioritising with the MDT.
• Carry a delegated caseload of clients with a range of needs and report progress to the MDT.
• Undertake skilled rehabilitation using innovation and adjust treatment plans with guidance from registered staff.
• Work independently to implement and evaluate occupational therapy and physiotherapy interventions.
• Provide feedback to the MDT on patient condition and actions required.
• Assist the MDT in evaluating intervention progress using outcome measures.
• Identify need for specialist equipment and adaptations to promote independence.
• Provide planned therapeutic activities; deliver treatments one-to-one or in groups.
• Complete risk assessments and monitor safety for patients, self and colleagues.
