Surrey Downs Health and Care in Epsom is seeking a Senior Rehabilitation Support Worker to support home-first discharge pathways and coordinate rehabilitation across community and hospital settings. The role sits within the Recovery At Home team, delivering timely care and assessment to enable safe discharge and ongoing needs evaluation.

The post involves working in a multidisciplinary team, with scope to develop skills and competencies in collaboration with physio, OT, nursing, and social care

Competenze

• Educated to NVQ 3 level or equivalent in a relevant subject.
• Diploma level or equivalent through short courses or experience.
• Intermediate IT skills with Outlook, Word, Excel; AIS and EMIS knowledge.

Mansioni

• Complete skilled screening assessment for clients referred to the service.
• Carry a delegated caseload of clients with a range of needs within a defined clinical area.
• Undertake skilled rehabilitation using innovation and report progress to the MDT.
• Work independently in implementation and evaluation of occupational therapy and physiotherapy interventions.
• Provide feedback to the MDT on changes in patient condition and relevant information.
