Job summary

Surrey Downs Health and Care invites applications for a Rehabilitation Assistant to join the Community Therapy Team serving the Cobham and Oxshott area. The role involves working with Physiotherapists, OT and nursing staff to deliver patient-centred rehabilitation in homes, clinics, and community settings.

Applicants should have GCSEs in Maths & English, a Care certificate, and access to a car. Prior community-based experience is advantageous; full training will be provided. Driving is essential.

Qualifications

• GCSEs in Maths and English or equivalent.
• Care certificate required.
• Experience in a team environment, preferably in NHS.
• Driving licence and car ownership for business use.

Responsibilities

• Carry own patient caseload under qualified therapists.
• Assist in mobility and transfer activities.
• Support clinicians with therapy programmes and notes.
• Liaise with patients, carers and family to explain exercises.
• Deliver therapy sessions in homes or community settings.
• Maintain accurate progress notes in EMIS / Systm1.
• Contribute to multidisciplinary team planning and CPD.
